eeloe asks About Stelara and Sickness
Right, so as i posted in my main thread, i was sick last week and ended up on Antibiotics, so couldn't take my dose of Stelara last thursday like I was supposed to.
Now, i'm going to take it this Thursday, but what i want to know is, should i take my next dose, in 12 weeks, or 11? should i stay on the same schedule, but just take last weeks dose a week late, or should i start a whole new 12 week cycle from this Thursday?
Read more Here Duncan asks about Otezla rash
Used Otezla for 5 weeks now. No bad side effects. Had them all very mild and all cleared.
I now have started with a bad rash. Mainly at the top of my legs. I also have it behind my knees, behind my elbows, under arms. Every where my skin folds ( other than top of legs). Didnt bother me at first. Slight itch. Its now a tad irritating, at times. Sometimes its like when you get burnt in the sun. Other times I forget I have it. Had it for about 2 weeks now. Telephoned Hospital and she said to continue if I can. It was the nurse I spoke to.
Today I feel a bit bad. Like mild sun stroke. ......

Cosentyx shows low immunogenicity in Psoriasis patients treated for up to five years
This study suggests immunogenicity incidence over 5 years of treatment was consistently below 1% in psoriasis patients treated with Cosentyx (secukinumab)
Secukinumab is a fully human monoclonal antibody that selectively neutralizes IL‐17A, a key cytokine involved in psoriasis (PsO) and psoriatic arthritis (PsA) development, and has shown rapid and long lasting efficacy and safety in the complete spectrum of psoriasis manifestations. Monoclonal antibody therapies may be associated with the production of treatment‐emergent anti‐drug antibodies (TE‐ADA) that can affect drug pharmacokinetics, diminish clinical responses via inhibition of target binding, or cause hypersensitivity reactions. Secukinumab exhibited minimal immunogenicity up to 52 weeks in patients with moderate‐to‐severe plaque psoriasis, as evidenced by TE‐ADA in <1% patients.........
Read full article Here Increased vascular risk in psoriasis patients with depression
Psoriasis is known to confer a higher risk of cardiovascular and cerebrovascular diseases. However, very few studies have investigated whether the development of depression in psoriasis patients may further increase this vascular risk.
We investigated the risk of ischaemic heart disease and cerebrovascular disease in Taiwanese psoriasis patients with and without depression.
A nationwide population‐based retrospective cohort study was performed using the National Health Insurance Research Database. We included 604 psoriasis patients with depression, who were matched to 2,416 psoriasis patients without depression (1:4 ratio). Relative risks (RR) with 95% confidence intervals (CI) were determined using the Cox proportional hazards regression model, with adjustment for demographic characteristics and comorbidities......

This study suggests that Mental Health Inventory (MHI‐5) is a reliable and valid instrument that can be used to effectively capture psychological distress in psoriasis patients.
Patients with psoriasis are at risk of a comorbid diagnosis of depression and/or anxiety. It is therefore essential for dermatologists to have valid and effective instruments that can screen and monitor depression and anxiety symptoms in psoriasis patients.
The aim of this study was to validate the Mental Health Inventory (MHI‐5) as a brief measure that can be used to evaluate psychological distress related to anxiety and depression in psoriasis patients.
The sample included 76 adult dermatological outpatients diagnosed with psoriasis. Participants completed the MHI‐5, the Hospital Anxiety and Depression Scale (HADS), and six subscales of the Self Compassion Scale (SCS). Confirmatory Factor Analysis (CFA) was applied to examine the factor structure of MHI‐5. Convergent validity was examined by applying correlations among all measures. Discriminant validity was examined by applying hierarchical regression models. Reliability was examined by calculating the Cronbach's alpha coefficient...........
Read full article Here Skyrizi gets FDA approval for treating psoriasis
AbbVie announced that the U.S. Food and Drug Administration (FDA) approved SKYRIZI™ (risankizumab-rzaa), an interleukin-23 (IL-23) inhibitor, for the treatment of moderate to severe plaque psoriasis in adults who are candidates for systemic therapy or phototherapy.3 In clinical trials, SKYRIZI produced high rates of durable skin clearance – most people (82 and 81 percent) treated with SKYRIZI achieved 90 percent skin clearance (PASI 90) at one year, with the majority (56 and 60 percent) achieving complete skin clearance (PASI 100).
"The complex nature of psoriasis and the variability or loss of treatment response over time can prevent some patients from achieving their treatment goals," said Kenneth B. Gordon, M.D., a principal investigator for the ultIMMa-1 pivotal trial and professor and chair of dermatology at the Medical College of Wisconsin. "In clinical trials, risankizumab demonstrated high levels of skin clearance that persisted through one year. I'm pleased the dermatology community now has a new option that can help patients achieve and maintain a high level of treatment response."
The recommended dose for SKYRIZI is 150mg – administered by two subcutaneous injections every 12 weeks following two initiation doses at week 0 and 4. SKYRIZI can be administered in-office or by self-injection after training. Read more Here

Fred's Music Column.
Indie Music: When you think of Indie Music (Independent music) most people thing of Rock or Pop, but there are a lot of different genres of music under the name "Indie" and the term is used all around the world. So what is Indie Music ? Indie music is music that is made without the backing of large record labels. You know ....... the ones that control what we listen to on the radio.
The term "Indie" can be traced back to the early 1900s where it was first used by Independent film makers, then in the 1970s the term started getting used in the music industry. Think "Virgin", "Mike Oldfield", "Tubular Bells" and you have probably the best known explanation of Indie in it's modern form. Call it D.I.Y (Do It Yourself) music if you like, it's a way of producers and artists getting their work out there without the backing of big money.
Those that go into Indie have to work very hard as the big radio stations either won't or can't play their music, and in the past a lot of Indie music never made it. But today things are different and it is a lot easier to find yourself some Indie music in a genre of your choice. There are dedicated internet radio stations, and a lot of artists will also put their work on Spotify, Deezer, Youtube, Etc. So go out there and find yourself some great Indie Music and support those that do it just for the music

Recipe Spot
Right then. So if like us you ended up with a leg of lamb that looks like it would have kept an elephant aloft you probably have plenty of lamb spare and are wondering what to do with it.
Hows this for starters? By the way our leg was roasted for around 2 hours with loads of garlic cloves rammed into slits in the leg and a good handful of rosemary rubbed on to it.
Lamb Tagine
Now this recipe officially calls for uncooked meat but the leftovers work just as well.
750g Lamb neck fillet hacked into 2 to 3cm cubes
2 Large Onions sliced
3 Celery sticks sliced
2 Garlic Cloves crushed
4cm lump of fresh Ginger peel and grate
1 tbsp ground Cumin
1 tbsp ground Coriander
2 tsp ground Cinnamon
1 tin chopped Tomatoes
450ml Chicken or Lamb stock
2 tbsp Tomato puree
2 tsp Harissa paste
1 tbsp Runny Honey
8 ready to eat Apricots quartered
2 Preserved Lemons, sliced into 8 chunks and the pips spat out ..... (Yep probably not that easy to get for some, so you can use a single fresh lemon but it wont be quite the same. And the skin isn't that edible when cooked where the preserved ones are fully edible.)
1 tin of Chickpeas (rinsed)
Handful of chopped flat leaf parsley. Always use flat leaf, the curly stuff should only ever be used by fishmongers as a display item, flat leaf has much more flavour.
Crank up that oven to 150ºC / 300ºF / Gas mark 2. (Does anybody still have a gas oven???)
Stick your best ovenproof frying pan (we use a nice heavy cast iron type thing with a lid) on the heat and if you are using fresh lamb start of by browning the chunks a few at a time so the pan temperature doesn’t drop and you end up inadvertently boiling you baa-lamb instead of browning it. Fry off for 3 or 4 minutes per batch and set aside.
Add a bit more oil to your pan and throw in your onions and celery, you want them softened but not coloured. Lob in the garlic and grated ginger, sprinkle over the spices cooking very briefly before throwing the tomatoes, stock, tomato puree, harissa paste, honey, apricots and which ever lemons you go for.
Bring this concoction up to the boil and return your lamb. Season with your favourite salt and pepper put its lid on and whack in the oven for an hour. Remove the lid throw in your Chickpeas, give it a quick stir, check its seasoning and continue cooking for another half an hour to an hour until everything is nice and tender. I have a feeling that this dish is virtually impossible to ruin, but I guess that somebody could prove me wrong. It will keep and reheat nicely and would freeze so you can enjoy later if you are into batch cooking.
Right guys I’m starving. Serve with a couscous salad flavoured to your liking, a nice fresh green salad and some warm flat breads.
Now I have given this some thought and I have not got a clue what to suggest to drink with it alcoholic wise. I suppose a cold lager type beer wouldn't do too badly, but I’m at a loss as to a wine.
Just a thought, an ice cold sweet lemon tea would be perfect.
Oh and the rest of our Elephant stump went into a hotpot. 1 Leg, 5 n’ a bit meals yum!
Enjoy
Johnb

How many places have you lived in ?
Heres a thread we can all join in, it's not difficult but makes you think and I'm sure others would be interested to read of the fact that perhaps you have lived in the same house all your life, or maybe you have moved lots of times and even changed countries come on in and have a look and join in if you like .......
.. Here Fred Asks How about we share our green tips. ?
I don't class myself as an eco warrior, but I try to do my bit and who knows some of you may have an idea that some of us could take on.
I'll start things off: Grow some of your own vegetables. And before you say I don't have room or time, just think back to those days at school when you grew your own little bit of Cress on blotting paper. Just one pot in your garden, balcony or even in your kitchen with one vegetable or herb of your choice growing in it will not only give you fresh vitamins it will also help fight pollution .................. Still not convinced ? Get yourself some sprouting seeds (Mung Bean are soooooo easy) but I bet you all buy them from a shop or your favourite take-away ?.......read more or join in

Losing my mind .....
I might be losing my mind
The words no longer flow
Repeating questions all the time
Forgetting what I should know
And when I use the wrong word
As I so often do
I’m not trying to sound absurd
I’m as surprised as you
Perhaps it is the memory holes
That cause the most upset
The things I’m told I said or did
I just can’t recollect
But then of course I’m tired
I don’t sleep that well at night
And my body’s busy fighting stuff
It doesn’t need to fight
Maybe it’s just advancing age
My grandad was the same
He’d go through every other one
Before coming to my name
I might be losing my mind
My words might be a hit and miss
But please don’t be unkind
I think I’ve always been like this!
