I've had PPP for about five years and about six months ago it became pretty much unbearable, so I caved in and told my dermatologist I'm ready to try the oral meds. Three months ago I started on Neotigason/Acitretin 25 mg for two weeks and then upped it to 30 mg until I saw the dermatologist again. The worst of the side effects were that my lips were peeling badly and the inside of my nostrils was incredibly sore, but my hand and foot were definitely improving, to the point where I can show my hand without the PPP being obvious.

Wasn't particularly happy with the coating on my palate after eating, some dizzy spells, very dry skin all over, itchy spots appearing on my legs, hair loss - most of the usual side-effects. I was advised to cut back to 25 mg to lessen the side effects, and that's helped. Still get patches on my legs that I never had before and they itch, and spots on my arms itch too, mainly in the evenings. I stuff QV intensive moisturiser up my nose at least twice a day now instead of on my hand and foot which helps the dry and sore nostrils. My lips have improved but still get dry. It's been quite something to see how much skin has peeled off both of my feet, top and bottom, in places I never had PPP. When my skin is wet it becomes disturbingly smooth and sticky, and I have to be careful in the shower that I don't slip on the tiles. My foot hasn't improved as much as my hand, but apparently feet take longer. I'm glad I can walk without a limp now, though!

I could do with some advice: My hair has been thinning and I'm worried enough about that, but it's my eyebrows! Although I'm a brunette, my eyelashes and eyebrows are quite light and the Neotigason seems to have thinned what little I had in the way of eyebrows to the point where they're not obvious at all. I always used an eyebrow pencil to fill them in to make my face look less 'blank', but there's not much left at all anymore. I'm wondering about costmetic tattooing to help my self-esteem but I'm worried that the effect of the Acitretin on my skin may cause more problems. Does anyone have experience in cosmetic tattooing, or general tattooing, while using Neotigason/Acitretin?

Welcome to psoriasisclub.

You seem to have some of the classic side effects of acetretin. I am glad for you that at least it is giving you some relief, despite these side effects.
I get your thing about your eyebrows. Myself I would not easily go to tattooing. Two reasons for that are that I don’t like tattoo's, well bit of henna may be nice but nothing definite, and second is that we people with Psoriasis do really have to be careful with our skin. A tattoo might be a trigger for Psoriasis to pop up at that location.

Enjoy yourself on the forum, usually there is someone here, certainly when you put the coffee kettle on in the bar. Big Grin

Welcome to the Acitretin clan  Thumb . All you describe , I am acutely familiar with, pealing feet and hands were epic and the dry lips were on the whole somewhat unpleasant but they were all worth putting up with for a flake and itch free life. I didn't suffer from the itch you describe but it is a known side effect. My hairdresser noticed the change in my hair, it went thinner and curly. I followed the same regime 25 - 30 - 25mg and up until getting an umpteen week headache was quite happy to continue. It was stopped by the Dermy until they had run some tests. I am now back on at 10mg daily and up to now no side effects. So back to your questions. I wouldn't be too worried as all the side effects do calm down eventually and all are reversible once your body gets used to the drug. As for the tattoo, personally I wouldn't. As Caroline has said any skin damage is a magnet for the Ps to take hold. Every burn scrape and graze I have ever had in the last 10 years that has not been on a hand has progressed to scale. I can understand that as a young lady, looks do matter, could you have the temporary ones done that don't break the skin?

Hi moonscribe and  Welcome to the club, I'm afraid I can't help with your acetretin query but I do know that several of our members do use it, I see JohnB has answered and he has posted a lot of his experience here

I just thought I would pop in and say hi and hope you get some helpful answers, but regarding the tattoos I would be wary as you might get a reaction and trigger psoriasis on your face.....if you decide on temporary tattoos make sure you go for natural henna not black henna which can cause nasty reactions .... but to be honest if I had the problem and was a woman , I think I would use an eyebrow pencil ....

If you put "acetretin" in the search facility    Search it will throw up lots of results

Hello Moonscribe  Welcome to Psoriasis Club.

I see John has answered and he knows more than me about Acitretin, so I'll leave you to it. But as you posted an intro I thought I would say hello.

As for tattoos, yes I would think twice. Not only is it permanent but it could end up giving you psoriasis where the tattoo is. Personally I would think of changing your treatment as it doesn’t sound like Acitretin is going to do you any favours.

All I can say is good on you for having the gumption to go on systemics. There are very knowledgeable and helpful people here with a wealth of experience. There are many good treatments available, so to each his own and better the devil you know.

I'm afraid I haven't any advice on ppp and I don't know how many p treatments are applicable to it, but personally I would mention to you derm that you don't like what's happening to your hair and eyebrows in case they can move you to another treatment? However as I say I don't know so much about ppp and maybe it's good to stick with the thing that works..

If the effects are reversible on stopping the drug, I don't think I would personally go for the tattoos. I imagine they are good if you have more permanent hair loss though.

JohnB - I'm actually not that young - I'm nearly 53 lol. I've got an appointment with a hairdresser after work for a cut and colour, something that will work with the thinning after having long hair for a few years, although I did have some chopped off recently to get used to the idea of short hair again!

Caroline - I've got several tattoos already, but I did get them before the PPP started on my hand and foot. I'm more worried about how my skin behaves on the meds now. The hairdressing salon I'm going to today also does the cosmetic tattooing so I'll pick their brains about the procedure, but will most likely err on the side of caution and stick with the pencil and kohl - also lost quite a few eyelashes last year after an operation to remove a lump on my eyelid *sigh*. Gosh it would be lovely to just wake up and go instead of trying to hide the worst of the imperfections!

My next appointment with the dermo is in about five months, so I'll keep persevering with the acitretin to see how it works over the longer term (encouraged that some symptoms have subsided). All the meds have drawbacks, so I guess it's just about finding the one with the least annoying side effects, if bloods, dermo and other considerations allow.
