Most modern healthcare systems are more concerned about short term costs over long term costs. It doesn't make sense not to provide effective treatment at the start since it's been shown to significantly reduced long term cost.

But the cheaper treatments can be extremely effective for some patients. With my latest 25 kg purchase, my annual cost for DMF is a few cents over $2 US. I would not change to secukinumab if it were offered me free of charge as I doubt it could be as effective; Even were it marginally better, I could not see the point of burdening taxpayers for a small advantage to me.

Maybe you could go to a fancy dress shop and get a derelict drug addict costume? Then you could queue jump above all those fine upstanding citizens like your good self.
