I wouldn't use typical steroid creams in the ear canal. That's a typical tightwad dermatologist for you (just mix it with water nonsense). If you have a bit an infection as well, theres several ear drops than can be used. If you can't get a dermatologist to subscribe it, maybe try a general physician.
