If he is the top dermatologist, you may find it difficult to get to see him. I see the top one at my hospital, but I have known her around 12 years and when I first met her she was not the top doctor.
Sounds like you would be better off trying elsewhere, or wait another week or so to see if the appointment turns up. Thought going by what you have said, I wouldn't bet on it.
