You are not going in there as a calm person discussing what colour to paint your lounge, this is an appointment to get help with something that is hugely affecting you and so yes, an appointment with a derm can be very emotionally charged. They have probably encountered other patients who are in this state as well as those who are on a stable treatment and feel more settled about the whole thing.
So I’m sure you can start afresh with her the next appointment.
Funny enough I had a bad experience with mine the first time we met. I didn’t see them again for a while, then my normal derm retired and I got them again. We get along just fine now.
