Caroline, I agree with you on the rheumatologists....mine has quite the reputation but he's the only one we have in the region on the Quebec side. 
Fred, I did do the loading doses, in November last year. The rheumatologist prescribed only 150mg. When I told him in August it wasn't working for the arthritis, he suggested to try something else. I asked to increase the dose instead. So here I am, three 300 mg doses taken so far and hoping this will work
He's keeping me on a lower dose of azathioprine because of the lupus I have....according to him.
