Let's say you had 1000 people with p who were unresponsive to topicals and only $2 million annually to spend on medication. Now if 70% responded to DMF powder, you might treat 700 for ~$2100. You could then treat 210 of the remainder with methotrexate for perhaps another $2500. You would then be left with over $20k per person for those unresponsive to either therapy.
Yes, my numbers are silly, but I just wanted to make the point that putting people trough a treatment sieve will at least allow better outcomes for the really tough cases. Public health is about giving the best outcome for as many as you can.
