I have only noticed it where I put the cream inbetween my toes, having said that it does look a slightly bigger area than where I apply it. However this could be because as I walk the cream gets squished around and spreads to areas I didn't put it.

You are obviously and "rightly so" concerned about using such a treatment in a delicate are. There are other creams you could try and personally I would prefer one that doesn't thin the skin. So yes, go and tell your dermatologist you are not happy and want to try something else.
