Of course doctors will ask you if you smoke it's bad for you and some studies say it can make psoriasis worse. I found giving up smoking made no difference at all to psoriasis or psoriatic arthritis. If you don't want to go to the doctors then you are going to struggle to live with psoriasis without doing so in my opinion.
You need a proper diagnoses and then a referral to a dermatologist and/or rheumatologist, they are better suited to help you and are the only ones that can prescribe something that will help. So bite your lip and get an appointment with your GP, answer his/her questions and go from there.
