With regards to the cream, I'd probably use something as long as it works. Sometimes you can persuade the doc to put things on your script but I don't think that kind of thing will be on their list of products.
I'd be asking for a referral, with that sort of coverage you should get seen and it's impractical to use creams on that large an area.
