Hi and a big welcome to the club 
it sounds as if it's been creeping up on you over the years which is very unfortunate, it's not something I have had. I have the ordinary pustular psoriasis. I do wonder have you seen a proper dermatologist who has a range of treatments for the disease and should be able to give something to make life more bearable
Have you used a good virgin coconut oil as a moisturiser, it's cheap effective and might stop the cracking plus you can pick it up at your grocery store
