Mine used to come away from the bed and were very painful before going on to Stelara. It took around a couple of years before they was back to almost normal.
Once a month I soak my feet in warm water with good old French sea salt for around 1 hour, then I clip the nails straight across and rub in some coconut oil.
Do not pick at them and don't be too tempted to clip them in odd shapes. Clipping the corners to keep sharp edges is fine but by keeping them straight the bad bits will eventually grow out and catch up. It takes time, but mine apart from a few little pits and one with a deep line are fine and no longer lift away.
If you haven't already got it you could be heading for psoriatic arthritis, so get your dermatologist to have a look as you may do better on Cosentyx if you do have onset psoriatic arthritis.
