I don't think it's uncommon for topicals to be ineffective, yet it seems to be the first step. I'm not saying they don't work, just that they don't work for everyone. It will most likely take getting in to see a dermatologist (as has already been said) and finding the right treatment that works for you. Usually treatment seems to be topicals, UV treatment, oral meds, biologics and it takes failing on one to move up to the other. But it all depends on doctors, insurance, where you live, etc but that seems to be the usual course that doctors follow.
